We offer assistance and support
Individual Community Living Support, or ICLS, is specially designed to provide support and assistance to your loved ones who need reminders, cues, intermittent or moderate supervision, or physical assistance in their homes. The people eligible for this service are those aged 65 and above who hold Elderly Waiver (EW) or use the Alternative Care Program.
As per the Minnesota Department of Human Services, Active Care can provide you with six service categories, namely:
Active Cognitive Support
- Solving the client’s concerns related to daily living
- Assuring the client
- Observing and redirecting to address behavioral, orientation, or other cognitive concerns
Adaptive Support Service
- Verbal, visual, and touch guidance to help a client complete a task
- Developing and demonstrating cueing or reminder tools
- Helping clients understand written assistive technology directions or instructions to help the client maintain independence.
Activities of Daily Living (ADLs)
- Assisting with self-care tasks, such as:
- Bathing
- Toileting
- Dressing and undressing
- Grooming
- Transferring
- Walking
- Climbing stairs
- Eating
- Mouth care
Household Management
- Cleaning, meal planning and preparation, and shopping for household/personal needs
- Financial management
- Assisting with communications (e.g., placing phone calls, making appointments, etc.)
- Setting transportation appointments integral to ICLS community engagement goals and community resources and if informal supports are unavailable.
Health, Safety, and Wellness
- Monitoring and recording health status and notifying case managers/family members about it
- Implementing changes to mitigate environmental risks at home
- Keeping track of health maintenance and improvement activities
- Providing medication assistance (e.g., refills, reminders, administration, preparation)
- Monitoring the client’s health status according to written instructions from a licensed health professional
- Using medical equipment or adaptive technology devices according to written instructions from a licensed health professional.
Community Engagement
- Facilitating the client in socially valued roles through engagement in relevant activities that lead to desired outcomes
- Helping the client access activities, services, and resources that facilitate meaningful community integration and participation
- Helping the client develop and maintain their informal support system
- Setting transportation appointments integral to ICLS community engagement goals and community resources and informal supports are unavailable.
